Frequently Asked Questions: Sports Medicine

Q: I want to come out for a varsity sport. What do I need to do to be cleared for participation?

A: All student-athletes that have not previously participated in a Varsity sport for F&M must complete the following:

  • A full physical performed by the Certified Athletic Training Staff and Appel Health Services Staff. Please fill out the Pre-Participation Physical Exam Form.
  • Insurance form with current insurance information and emergency contact numbers. Please fill out our insurance requirement. 
  • All of the above steps must have been completed before you may participate in practice or competition with your team.

Student-athletes that have previously participated in a Varsity sport for F&M must also have a sports physical. Sports physicals need to be completed on campus and will be scheduled through your coach

 

Q: I want to come out for a club sport. What do I need to do to be cleared for particiption?

A: All students who want to participate in the following clubs; men's rugby, women's rugby, men's crew, cheerleading, and ice hockey must complete the following:

  • A full physical performed by the Certified Athletic Training Staff and Appel Health Services Staff. Please fill out the Pre-Participation Physical Exam Form.
  • Insurance form with current insurance information and emergency contact numbers. Please fill out our insurance requirement. 
  • Medical history forms will be reviewed sports medicine and appel helath services staff.  If a physical is required, you will be contacted within 72 hours.
  • All of the above steps must have been completed before you may participate in practice or competition with your team.

 

Q: Are there student jobs available in the athletic training room?

A: If you interested in a student assistant position in the athletic training room, please contact Matthew Keller, Associate Athletic Trainer, to check on openings that we may have. Feel free to contact us if you have general questions. There is a one credit Sports Medicine internship that students interested in allied health or medical professions may be interested in. Q: I have recently had surgery, a visit with a physician, physical therapist, chiropractor or other allied health professional back home. What paperwork do I need to provide to be cleared for participation in a varsity or club sport?

A: One of the issues that sometimes arises during the year is that of surgery that has occurred away from campus.  If this is your case you need to supply some information so that we are fully updated to your situation. Items such as surgical reports (ie. what the surgeon did), follow-up notes from the physician noting progress or return to sport. Please understand that simply providing a note that states you may return to sport in place of the above paperwork is not sufficient. The college physician has the ultimate say as to your participation in a varsity or club sport.
